What do I need to send?
The bill, and the insurance statement for that visit if you have one. Cover every account and member number except the last 4 digits. Never send your full Social Security number.
Is my information private?
Yes. Your files go in a private folder shared with your email only and are deleted 90 days after the task is finished.
