How to Get an Itemized Medical Bill and Check It
A summary bill shows one big number. The itemized bill shows every charge, and that is where errors show up. Here is how to ask for it and what to check.
1. Gather what you have
- The bill from the hospital, doctor or lab, with the account number and dates of service
- If you have insurance, the Explanation of Benefits (EOB) from your health plan for the same visit. It isn't a bill, but it shows what the plan says you owe.
- Your own notes: when you arrived and left, and what tests or treatments you remember
If you have insurance and the EOB hasn't arrived yet, wait for it before you pay.
2. Ask for the itemized bill, in writing
Use the patient portal message center or the billing office's email or mailing address, and keep a copy:
Hello, I'm reviewing my bill for account [number], for services on [date(s)] for [patient name]. Please send me a fully itemized bill showing each charge, the date, the billing code, the quantity and the amount, plus all payments and adjustments from insurance. Please also send your financial assistance (discount payment and charity care) policy and application. While I review the bill, please don't send the account to collections. Thank you, [Your name] [Phone or email]
3. Check it line by line
- Duplicates: the same test, medicine or supply billed twice on the same day
- Services you didn't receive: a test that was ordered and then cancelled, or a visit that didn't happen
- Wrong dates or quantities: extra days in a room, or charges after you went home
- Insurance not applied: payments and adjustments on your EOB that are missing from the bill
- Your details: your name, date of birth and insurance ID are correct. A typo can cause a claim to be denied.
If something doesn't look right, write back with the line, the date and the amount, and ask for a corrected bill. If a charge was billed to insurance with a typo, ask the billing office to fix it and resend the claim.
4. In California, ask about hospital financial assistance
California hospitals must offer discount payment or charity care to patients who are uninsured or have high medical costs, with family income at or below 400 percent of the federal poverty level. You can apply at any time, even after a bill has gone to collections. If you are found eligible and already paid too much, the hospital must refund the difference within 30 days. These rules cover hospitals; separate bills from doctors or labs may follow different rules, so ask each one about its own policy.
If a hospital wrongly denies your application, or didn't tell you about its programs, you can file a complaint with the state's Hospital Bill Complaint Program. It doesn't handle general billing disputes.
5. Credit reports and insurance
- Credit reports: in California, medical debt is not allowed on your credit report (since January 1, 2025). If you see it, tell the provider, whoever holds the debt and the credit bureau. You can also report it to the California Department of Justice.
- Insurance denials: if your health plan refused to pay, that goes through the plan's own appeal process. For many California health plans, the DMHC Help Center (1-888-466-2219) offers free help.
6. Keep a simple record
Save every bill, EOB, letter and reply, and write down the date of each call or message and the name of anyone you dealt with. Pay only the amount you agree with once the bill is corrected, and keep the receipt.
General information only, not legal or financial advice. Every company and card issuer has its own rules.
